With Family First Direct Primary Care, you not only get exceptional care – you get

You may schedule a free 15 minute phone consultation with Dr. Gupta. She will review your individual healthcare needs, answer any questions you have about her direct primary care practice, and help you decide if joining her practice is the right decision.
An enrollment fee of $125 per adult or $75 per child under age 18 will be added to all initial invoices.
Patients are billed monthly and may discontinue their membership with 30 days notice and a signed cancellation form.
Re-enrollment after cancellation is $175 per person and is not guaranteed.

We also work incredibly hard to obtain affordable pricing for services that need to be handled outside our office. We share these wholesale prices with our patients, so that they can stop worrying about medical bills and focus on keeping themselves and their families healthy.
