You may schedule a free 15 minute phone consultation with Dr. Gupta. She will review your individual healthcare needs, answer any questions you have about her direct primary care practice, and help you decide if joining her practice is the right decision.
An enrollment fee of $100 per person will be added to all initial invoices.
Patients are billed monthly and may discontinue their membership with 30 days notice and a signed cancellation form.
Re-enrollment after cancellation is $200 per person or $400 per family and is not guaranteed.

Your monthly membership includes a wide range of medical services, without any copays or additional fees. We also work incredibly hard to obtain affordable pricing for services that need to be handled outside our office. We share these wholesale prices with our patients, so that they can stop worrying about medical bills and focus on keeping themselves and their families healthy.
*Pricing may vary based on which STI tests are ordered. Patients will be given pricing up front and also given the option to decide which tests should be done.
